(Rochester, NY) - Catholic Charities Family and Community Services (CCFCS) is pleased to announce that Peter Bauman, LCSW-R, PMP, has joined the agency as Chief Administrative and Process Officer. Peter comes to CCFCS from the 360 Collaborative Network, which he led as part of his role at Finger Lakes Performing Provider System (FLPPS) as Senior Director of Greater Rochester Health Home Network (GRHHN).
Peter has a 25+ year history in the health care and social work field, with the last 9 years at FLPPS and GRHHN. Prior to FLPPS, Peter held various leadership positions at managed care organizations. He utilized his expertise to implement workflows and processes to improve patient quality of care by increasing patient and provider engagement. Peter serves on the NYS Health Home Coalition, providing guidance to the health home advocacy group and fellow lead health homes. He is a Licensed Clinical Social worker who earned his Project Management Certification in 2010 and is Certified Green Belt Lean Six Sigma.
Peter brings to CCFCS a strong background in project management, knowledge of health care systems and value-based care, and a deep understanding of the vital role of social care services in positive health outcomes for the region.
