Our services for seniors include programs that enable folks to live confidently on their own when and where appropriate. We help them find subsidized home care and transportation to medical appointments, grocery shopping, and other essential errands.
Helping seniors stay independent in their own community as long as possible supports their human dignity, avoids the costs and challenges of moving to higher levels of care too soon.
The Nursing Home Transition and Diversion (NHTD) Waiver is one of the options available to New Yorkers with disabilities and seniors so they may receive services in the most appropriate, least restrictive setting.
