Expanded In-home Services for the Elderly Program

Helping seniors maintain independence

Since 1987 Family and Community Services has been administering Monroe County’s EISEP collaboration. This is a state and county funded service to help people stay at home and avoid needing to use Medicaid by premature moves to a higher level of care. It provides case management and subsidized home care and other services to help the older person stay independent in the community.

This program is funded in part by Monroe County Office for the Aging, New York State Office for the Aging, United Way of Greater Rochester, US Administration on Aging, and participant contributions.

What the Program Offers

The EISEP Community Collaboration has case managers at four different sites - Catholic Charities Family and Community Services, Urban League of Rochester, Lifespan, and Jewish Family Service. Our county is fortunate to have the support of the United Way of Greater Rochester and the Finger Lakes to offer these very special services to our older and vulnerable adults.

Case Management, Subsidized Personal Care/Housekeeping aide service, and other possible supports such as subsidized Social Adult Day Care, a Personal Emergency Response System (PERS units), home delivered meals, home repairs etc...

Eligibility criteria

Individual must be over the age of 60 and living in Monroe County. He/she cannot be eligible for Medicaid and must have a functional need for assistance with activities of daily living (bathing, meal prep, laundry, housekeeping, dressing, mobility).

Call (585) 262-7060 to speak to our Intake Coordinator if you or someone you know might benefit from these services. Please note: this is a voluntary service so the senior must be willing to accept the help.

Program Cost

No charge for case management, but the subsidy for any direct services is based on a person’s monthly income. A state-wide formula determines the cost share. About 60% of our participants are "donation based," or funded through donations, and do not have to pay a fee for the in-home supports.

A person only has to share their financial information if they want to use the subsidy.
