Program Overview
Family and Community Services coordinates and manages care for adults with chronic illnesses including mental illness, substance abuse, asthma, COPD, diabetes, heart disease, high blood pressure and obesity, etc. We support these individuals by –
- connecting to Medicaid - we help eligible individuals connect to needed services and supports in the community, including, but not limited to, benefits, medical, housing, financial, vocational, educational, and more.
- focusing on a person’s strengths and abilities, and identifying the supports needed to improve their overall quality of life and health.
- working collaboratively with all supports to ensure effective, person-driven services of the highest quality.
